REQUEST FOR PROPOSAL: Community Health Needs Assessment (CHNA)
November 2024
The Lake Shore Hospital Authority is requesting proposals from Consultants to perform a Community Health Needs Assessment (CHNA) survey and report for the LSHA Service area. CLOSING SUBMISSION DATE PROPOSALS must be received no later than 11:00 am (EST) on January 17, 2025.
Any questions received, and all answers provided have been attached to the RFP starting on page 4.

Welcome to the LSHA
The Lake Shore Hospital Authority (LSHA) oversees maintenance and operation of Shands Lake Shore Regional Medical Center in Lake City Florida. It has responsibility for ensuring that the hospital provides medical care options to the residents of this region and providing assistance to the indigent. The LSHA serves a region with a growing population and a dynamic economy. Thus, the LSHA must be proactive in anticipating and preparing for the changing medical needs of this region. There is the recognition that the hospital must grow and develop in order to meet the medical needs of the community.
The LSHA also acknowledges and embraces the role it plays in the betterment of the community. It is through thoughtful, well-conceived actions that the Lake Shore Hospital Authority will fulfill its mission and achieve its vision. These steps are articulated in a strategic plan for hospital maintenance, development and growth.
Eligibility Hours: Monday – Friday from 8:30 am to 4:00 pm
